We have an online Patient Portal where you can request your medical records, as well as view your previous lab results and patient plans.
To request your medical records, simply log into the portal using the button below. Then click on “Labs and Documents” and then “General Resources for All Patients” to download the medical records release authorization form.
Once the form is complete simply scan or fax it to (512) 721-0248.
There is no charge when we fax your files directly to another practitioner.
